Agreement of Release and Waiver of Liability

I hereby agree to the following:

1. I am participating in the classes and/or shows or performances offered by the Elevation Teachers as part of the Elevation Belly Dance Extravaganza, which may include any or all of the following: Carolena Nericcio, Donna Mejia, Ebony Qualls, Elizabeth Ashner, Frank Farinaro, Michelle Sorenson, Moria Chappell, Elizabeth Fish, Zoe Jakes and any employees thereof, other teachers not specifically named herein, as well as any of the above teachers employees, agents, subcontractors, or affiliated companies (Elevation Teachers).

2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the classes. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the class, health programs, workshops or performances.

3. In consideration of being permitted to participate in the classes and/or shows or performances I agree to assume full responsibility for any risks, injuries, damages or death, known or unknown, which I might incur to my person or property, as a result of participating in the program.

4. In further consideration of being permitted to participate in dance classes, I knowingly, voluntarily and expressly waive any claim I may have against the Elevation Teachers, Elevation Volunteers, Elevation Belly Dance, LLC , Elevation, the owner Elizabeth Harnisch, Muse LLC, their Employees, agents, and subcontractors, The Golden Community Center, The Buffalo Rose, Table Mountain Inn, The American Mountaineering Center, and The City of Golden for injury or damages to my person or property that I may sustain as a result of participating in the program.

5. I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue any of the aforementioned Elevation Teachers, including those instructors not specifically named, Elevation Belly Dance LLC, Elevation Teachers, Elevation, the owners Elizabeth Ashner LLC their Employees, agents, and subcontractors, The Golden Community Center, The Buffalo Rose, Table Mountain Inn, The American Mountaineering Center and The City of Golden for any injury or damages that I may sustain as a result of participating in the program, including any injury caused by their negligence or other acts or omissions.

6. I understand that during these classes the teacher may occasionally touch me to guide and assist my movements, and give my consent thereto.

7. I understand that during a show or performance, or during social breaks including dining, alcoholic beverages may be present and available for consumption. I understand that the consumption of alcoholic beverages prior to my own performance will inhibit such performance and may increase the risk of injury.

8. I understand and consent to the videotaping and photographing of myself during classes, shows, or performances, both as an audience member and as a performer, and agree to allow the use of such videos and photographs for promotional purposes by Elevation Belly Dance Extravaganza and Elevation Teachers.
